This is the stock xorg file for the new versions of xorg which attempt
to self-configure.  Xorg will determine the video resolution by querying
the monitor and then attempting to use its prefered resolution.  If, as
you say, you are using an older monitor, it is possible that it is
reporting the wrong resolution, or failing to report any resolution at all.

You might want to check your /var/log/Xorg.0.log file for something that
looks like the following.  If there is nothing like it, or it reports
strange values, this could be the source of your problems, in which case
you may need to manually tune your xorg.conf.
